Contact us
There are three numbers and addresses attached to this product, doing three different jobs. This page sorts out which one you need.
For orders, tracking, changes and refunds, call the order desk on +1 (302) 200-3480, Monday to Friday, 9:00am to 5:00pm Eastern Time.
For anything about this website — a mistake on a page, a typo, a privacy question, press — email [email protected]. The number printed on the bottle, +1 (623) 377-4609, belongs to the distributor named on the panel and is a different desk again.

The number on the bottle
+1 (623) 377-4609

Printed on the Supplement Facts panel as the distributor's contact.
A different company from this retailer
Why there are two phone numbers
This site is the retailer. The company named on the panel is the distributor. They are different organisations with different records, and a buyer holding the bottle will see the label number first.
If you bought through this site, the order desk is the one that can find your order. The distributor cannot.
The addresses
- Office
- 32 Avenue of the Americas, Floor 22, New York, NY 10013, USA
- Returns
- 590 Madison Avenue, Floor 21, New York, NY 10022, USA
- Distributor on the label
- Amplify Distribution, LLC, 7904 E Chaparral Rd STE A110618, Scottsdale 85250 AZ US
- Ships from
- The United States
Do not post a return to the office
The office address and the returns address are different, and a parcel sent to the wrong one will not reach the returns process. Get a return authorisation from the order desk first, and use the address they confirm.
What to have ready before you call
Four things that turn a ten-minute call into a two-minute one.
- Your order number, from the confirmation email the checkout sent you. This is the single thing that speeds up every order question.
- The email address you ordered with, which is not always the one you are writing from.
- The lot code from your bottle, printed on the base. The reference supplied to this site is COG-26/GO-2760, but check your own.
- The date the parcel arrived, roughly. Return windows are counted from delivery rather than dispatch, so both dates matter.
If you are writing about something that arrived damaged, a photograph attached to the first email is worth three paragraphs of description. Send it with the first message rather than waiting to be asked.
What this desk can and cannot do
| We can | We cannot |
|---|---|
| Find your order and its tracking | Tell you whether this will work for you |
| Authorise a return and confirm the address | Advise on interactions with your medication |
| Tell you what the label prints, exactly | Tell you the split of the 635 mg blend — we do not have it |
| Correct anything wrong on this website | Interpret a symptom, or offer medical advice of any kind |
| Pass a question to the distributor | Speak for the manufacturer about formulation decisions |
The right-hand column is not evasion. Questions about medication and symptoms belong with a clinician who can see your history, and we would rather say so than guess at them.
One thing we will always answer
Any question about what the label says. If you want a line from the panel read back to you, or a comparison between what your bottle prints and what this site transcribed, that is a two-minute call and we are glad to have it.
Reporting a problem with the product
- If the safety seal is broken, do not take any of it. Photograph the seal and contact the desk the same day.
- If the bottle or the parcel is damaged, photograph both before you unpack further.
- If the back panel does not match the transcription on this site, photograph it and call before opening the bottle.
- If you experienced something you think was a reaction, stop taking it, talk to a clinician, and tell us. You can also report an adverse event to the FDA through its MedWatch programme.
We would rather hear about a problem than not, including one that reflects badly on the product. A complaint we do not know about is one we cannot fix.
Response times
- Order desk, by phone
- Answered during Monday to Friday, 9:00am to 5:00pm Eastern Time
- Website email
- Usually within one working day
- A correction to a page
- Checked the same week; published as soon as it is confirmed
- A privacy or deletion request
- Handled inside thirty days
- A return authorisation
- Issued on the call, in most cases
Outside those hours, email is the better route and it goes to a real inbox. If an order question is urgent and the desk is closed, write down your order number and call when it opens; almost nothing about a supplement order gets worse overnight.
Order CogniMary from the official website
Sixty capsules, thirty days, and a panel you can read before you buy. Start with one bottle so you can see the price and the guarantee term before you commit to six.
No price was published to this website. The figure is on the checkout page.

Writing a useful email
Most support exchanges that take four emails could have taken one. The difference is almost always what was in the first message.
Put the order number in the subject line
Not “question about my order”. The number, and three words. It gets the right record open before anyone reads a sentence.
The single biggest improvement
Say what you want to happen
A refund, a replacement, a tracking update, an answer. A message that describes a problem without saying what would resolve it takes an extra exchange every time.
Ask for the outcome
Attach the photograph first
Damaged parcel, broken seal, a panel that does not match: send the image with the first message. It is worth three paragraphs.
Do not wait to be asked
Keep it in one thread
Reply to the existing message rather than starting a new one. The history is what makes the second exchange fast.
One thread, always
None of that is about being polite to us. It is about getting a resolution in one round instead of four, which is worth more to you than to anyone here.
What we cannot answer, and who can
| The question | Who can answer it |
|---|---|
| Will this interact with my medication? | A pharmacist or your prescriber, with the panel in front of them |
| Should I take this for a diagnosed condition? | A clinician. This is a dietary supplement |
| How much of each blend ingredient is in a serving? | The manufacturer. We asked; we were not given it |
| Which species is the bilberry? | An identity test. The panel contradicts itself |
| What is my blood level of B12? | A blood test, not a product page |
| Why is the folate conversion wrong? | The formulator. We publish the discrepancy and the FDA rule |
We will pass any of the last four to the distributor if you want us to, and we will publish the answer if one comes back. The first two are not ours to answer and we would be doing you harm by trying.
Accessibility and this website
If any part of this site is hard to use, we want to hear about it specifically rather than generally.
- The panel transcription is real text rather than an image, so it can be read by a screen reader, enlarged without blurring, and copied.
- Every interactive element is reachable and operable by keyboard, and the focus outline is deliberately visible rather than removed.
- The site works with JavaScript disabled. Nothing is hidden and the FAQ still opens, because it uses native disclosure elements.
- Animation respects the reduced-motion setting in your operating system. With it on, nothing on this site moves.
- Colour is never the only way information is conveyed.
If something on this list is not true on your device, that is a bug and we would like the details: [email protected] with the page, the browser and what happened.
When to call rather than email
Both routes work. One is faster for some things than others.
- Call for a return authorisation, a live order problem, a delivery that has stalled, or anything where a decision has to be made in the conversation.
- Email for anything with a photograph attached, anything about this website, a correction, a privacy request, or anything you want a written record of.
- Either for a question about what the label prints. That one is two minutes on the phone or one reply by email.
If a matter is important, do both: call, then email a one-line summary of what was agreed. That is not distrust, it is how everyone avoids remembering the same call differently in three weeks.
